Insurance

We are starting a new policy within Co. A  We are requiring anyone who is in the unit to have with them the new medical info card.  Please take a few minutes and fill out the information on the online form.  This information may make a difference receiving medical treatment in an emergency. 

Please remember: If you don't have your card on you, you can't go on the field.  No Exceptions.

Under medical problems just put what is important (ie, Cop D, CHF, diabetes, etc.)
